This is the perfect role for an existing Care Co-ordinator, Care Supervisor or a Care Assistant looking to progress their career!

The Co-ordinator holds one of the most important positions in First City as all your actions directly affect the happiness and wellbeing of both our customers and our staff.

The purpose of the Care Co-ordinator role is to make sure our customers receive excellent quality care and support to help them remain safe and comfortable in their own homes.

You will be joining a team of friendly and kind-hearted professionals who are proud of the work they do, We provide training and ongoing support to ensure you understand our carers and our customers. Our Care Co-ordinators are part of the care team and would be expected to help with care calls where necessary and partake in out of hours on call duites.

Location: Swindon

Hours: Monday-Friday 08:30am -17:00pm. Some flexibility in hours may be required to suit the needs of the business

Salary: £23400 per annum, additional earnings available when partaking in out of hours duties

Key responsibilities of a Care Coordinator:

  • Allocation of care workers to customer visits using effective rostering, ensuring continuity of care and considering locations, skills required.
  • Communicating regularly with care workers, providing guidance and resolving queries in a timely manner
  • Maintaining telephone contact with customers and staff members
  • Ensure telephone calls are answered in a timely, polite and professional manner
  • Ensure all paperwork is completed and entered onto relevant systems
  • Support with the delivery of the on-call rota, dependent on the needs of the business. This will include being the designated contact for the out of hours teams during evening/weekends and supporting all aspects of care delivery.
  • Support the field team covering customer visits to support the business needs when required.
  • Be the 1st line of support for carers and clients with care related enquiries
  • Ensure compliance is met within the office and the care calls
  • Assist and support the manager
  • Maintain integrity when dealing with confidential matters
  • Provide supervisory support and instruction to care staff
  • Adhere to company policies
  • Approach clients with understanding and empathy, remembering that each client is to be treated as an individual
  • Work as part of a team that revolves around person-centred care and support

The successful candidate will:

  • Be computer literate and have good office skills
  • Have excellent customer service skills
  • Have an empathetic and a caring nature
  • Be organised and can work calmly under pressure
  • Have experience of working in various Healthcare settings
  • Be a people person with good communication skills
  • Be a team player
  • Support with other tasks required to meet the needs of the business.

The following would be beneficial:

  • Health and social care qualifications (NVQ 2/3 or above)
  • Valid driving licence with access to a car

Children's Support Worker

First City has new amazing opportunity's to support young people out in our community after school hours, around term times and Weekends. We currently support children from the ages of 6 to 17 years. We are committed to the safeguarding of children and young people and everything we do is designed to promote the safety and well-being of the children and young people we work with. We support those with Autism, Epilepsy, Cerebral palsy, Global developmental delay, Challenging behaviours and learning disabilities.

Location: Swindon and surrounding area

Hours Available: Full-Time, Part-Time

Salary: £13.72ph

  • Provide support with all aspects of their daily lives and encouraging them to engage with positive activities

  • Working with children to gain their independence skills both within the home and community
  • Working with children to meet their individual outcomes
  • Be able to write clear daily reports, incident reports and accident reports.
  • Administer medication as required (full training will be provided).
  • Support with personal care (washing, toileting, etc)
  • Be part of a team of professionals delivering high quality person-centred care, led by an experienced management team.
  • Assist in the increase of support during Half terms and Holidays

First City Nursing and Care have many bespoke opportunities for experienced support workers to support our intermediate and reablement services. This position is unique to a traditional domiciliary care role, as you hold further responsibilities, including liaising with other healthcare professionals to review a person's support on an ongoing basis, ensuring a holistic approach with a focus on working closely with our clients to achieving the person's desired outcomes.

No day will be the same as you join an outstanding team of highly trained individuals to promote and encourage independence to customers within your local community. We are proud to support other healthcare organisations including the local hospital by enabling individuals to return to their own home; with a range of community therapy, community nursing, frailty, housing and social care services, to promote independent living, support and plan their return back into the community, when hospital admission is no longer required.

A full, valid driving licence and access to your own vehicle is essential for this position as you will be traveling within the community to support individuals in their own homes.

The successful applicant will:

  • Hold at least an NVQ 2 in Care or equivalent - this is desirable but not essential as full training can be provided to work towards this qualification
  • Hold a minimum of one year's experience in health and social care (within a community-based setting would be preferable)
  • Hold a full, valid driving licence and have access to your own vehicle
  • Promote and encourage independence in a person centre approach, working with customers to achieve their outcomes and what matters to them
  • Have excellent communication skills and be confident to liaise with other healthcare professionals to advise on and support customers' requirements, which can include equipment & medication arrangements
