Job Description

A Community services coordinator is a bespoke opportunity for an experienced care worker to support our intermediate and reablement services. This position is unique to a traditional Care Coordinator, as the role is to support other healthcare organisations including the local hospital by enabling individuals to return to their own home; with a range of community therapy, community nursing, frailty, housing and social care services, to promote independent living, support and plan their return back into the community, when hospital admission is no longer required.

Location: Swindon

Salary: £24,960, plus additional payments when partaking in shared on call service of up to £2,520 per annum.

Hours: 40hrs per week shared on call service

You will be required to assist the staff within your multidisciplinary team to liaise with other healthcare professionals to review a person's support on an ongoing basis, ensuring a holistic approach with a focus on working closely with our customers to achieving the person's desired outcomes.

Alongside this, you will be responsible to support your lead in the allocation of care workers to customer using effective rostering, ensuring continuity of care & considering locations, skills required. Communicating regularly & providing guidance & resolving queries in a timely manner. Ensure all paperwork is completed and entered onto relevant systems. Maintain integrity when dealing with confidential matters. Approach clients with understanding and empathy, remembering that each client is to be treated as an individual.

Essential for role:

  • Hold a minimum of one year's experience in health and social care (within a community-based setting would be preferable)
  • Health and social care qualifications (NVQ 2 or above)
  • Be organised and work calmly under high pressure
  • Be confident to liaise with other healthcare professionals to advise on and support customers' requirements, which can include equipment & medication arrangements

Desirable:

  • Valid driving licence with access to a car
  • experience of working in various Healthcare settings

    Hours: Mornings with 7am starts and/or Evenings with 11pm finishes

    We support those with; Autism, Epilepsy, Cerebral Palsy, Global developmental delay, Challenging behaviours, Learning disabilities, Dementia, Parkinson's, Strokes, Huntington disease and Mental health challenges.

    Full driving licence and access your own vehicle is essential for this role

    Duties can include-

    • Assisting with personal care such as showering, bathing, incontinence care
