A new type of care.

First City Care Group is working to become a new type of community care organisation. We are committed to working in partnership with organisations and residents within a health and care system. Our singular focus is to enhance the wellbeing of the people we support and contribute towards the delivery of population health outcomes.

We work with other community care providers and aim to facilitate collaboration within local marketplaces. This collaboration ensures providers can work together to deliver the highest standard of services, maintain sustainable business models and have the greatest impact with the people we support.

Working as part of Integrated Care Systems

First City Care Group operates as active participants within integrated care systems. We know that without community care people cannot get the support they need to recover at home and that this has an impact on local acute services. We are eager to work with health and care partners to develop integrated services which ensure people are supported on the most appropriate care pathway.

Our model of provider-led brokerage is based on facilitating collaboration across community providers to ensure there is adequate supply of support staff to meet the needs of residents. We have developed a proactive brokerage model based on forecasting supply across the market and matching this to known and expected demands.

In addition, we work with other partners to support the delivery of population health and socio-economic goals. Examples of this include; working with integrated care systems to host roles focused on encouraging people into the community care sector, and working with public health to engage with elements of the Community that are traditionally underrepresented within the care sector.
