The first question you might have is what does S.H.A.P.E stand for?

Strength-based, Holistic, Approach/Aspirations/Assessment, Promoting, Enablement

Our S.H.A.P.E colleagues (formerly referred to as Support Planners) create each customer's person-centered support plan (we previously discussed what person-centered support means here). It is their responsibility to discuss and assess the individual outcomes of each person, what they need support with, what they hope to achieve and what they are able to do independently in order to create their bespoke support plan.

The support plan is written clearly and meticulously by the S.H.A.P.E team, including all risk assessments they have undertaken, required forms (including up to date medication sheets, hydration charts...etc). and accurate and up to date information. The support plan is a live document, which constantly needs to be reviewed and amended following feedback from Care Workers, other industry professionals, the customer and further risk assessments.

Collaborating with other professionals across the health and social care industry, the S.H.A.P.E team would ensure appropriate assistive equipment and technology are sourced and implemented safely for our customers who require it.

Empathy and compassion are essential within this role as our S.H.A.P.E colleagues would not only support customers through the implementation of their support but also explain to their loved ones, family, friends, or next of kin the process and help all those involved to adjust to what can seem daunting to some.

It takes a passionate person to be a member of the S.H.A.P.E team; someone who is kind, caring, compassionate, honest and truly wants to make a difference to the lives of others. Though it can be a tough role, sometimes with long hours, they see the fulfilment and career satisfaction, knowing they have played a part in making a difference to so many people in our local community.
