    In this role you will be working closely with the client and their care team to create and tailor their own package of care.     As a SHAPE facilitator you will be required to attend assessments and rebuilds with the client and their families and liaise with a variety of professionals across the health sector. First city works closely with a vast array of clients and services so adaptability is a must.

    For this role you must have a background of at least 12 months in care delivery, hold a level 2 qualification, equivalent or above in health and social care and you MUST drive with access to your own vehicle.
