Job description

Join our dedicated team supporting nearly 600 people across Swindon and the surrounding areas, playing a key role in delivering essential social care services. We are seeking a proactive and organised Care Coordinator to join our fast-paced office team. We have 2 NEW permanent positions, offering an excellent opportunity to grow your career in health and social care while working in a supportive and dynamic environment.

Location: Swindon
Salary: £26000 with an OTE of £29,500 (inclusive of on-call duties)
Hours: 40 hours per week shared out-of-hours on-call service
(Office hours: Monday-Friday, 08:30 - 17:00. Some flexibility may be required to meet business needs.)

Key Responsibilities:

  • Efficiently schedule and allocate care workers to client visits, ensuring continuity of care and appropriate skill matching.
  • Maintain effective communication with care workers, providing support, guidance, and timely resolution of queries.
  • Collaborate with healthcare professionals to ensure seamless service delivery.
  • Accurately complete and maintain all necessary documentation and systems.
  • Handle confidential matters with professionalism and integrity.
  • Provide compassionate, empathetic, and person-centred support to clients.

Essential Requirements:

  • Previous experience in health and social care, preferably in a community-based setting.
  • Strong administrative and computer skills.
  • A professional, caring, and respectful approach.
  • Excellent organisational skills with the ability to work effectively under pressure.

Desirable:

  • Health and social care qualifications (e.g., NVQ Level 2/3 or above).
  • Experience in rostering and scheduling.
  • Previous experience in an on-call capacity.
  • A valid UK driving licence and access to a vehicle.

Care Assistant - Independent Living Hubs (No Driving Required)

Up to £12.21 per hour Full training, pension

Are you caring, reliable, and passionate about helping others live independently? First City care group is looking for compassionate Healthcare Assistants to join our team within our Independent Living Hubs across Swindon.

These Hubs are residential buildings where individuals live independently in their own flats and receive support based on their needs. You'll provide care within one building at a time, moving from flat to flat - making this a great role for non-drivers. While a driving licence isn't required, there is an expectation of flexibility to work across our four locations in Swindon, currently with a high demand in Highworth and Wroughton.

You'll also have opportunities to support at on-site Day Centres, where individuals come to socialize, have meals, and enjoy activities.

Location: Highworth, Wroughton, Swindon
Job Type: Full-time or Part-time
Hours : 7am - 15:00pm / 15:00pm - 23:00pm/ 20:00pm - 7:00am
Salary: £12.21 per hour

Key Responsibilities:

  • Provide person-centred care based on individual care plans
  • Support with personal care, mobility, and medication
  • Help with daily routines, meal prep, light domestic tasks, and shopping
  • Promote independence and dignity

Some individuals may have multiple visits a day, while others require less frequent support, with visit durations ranging from 30 minutes to 4 hours depending on individual care needs.

We are looking for someone who is:

  • Able to arrange their own transport to and from shifts (especially important for early starts and late finishes)

Experience is not essential - full training is provided. We also welcome experienced carers and those who've supported family members.

Experienced Community Carer - Full UK Driving Licence & Own Vehicle Required

About the Role
This is a community-based position where you will support individuals recently discharged from hospital, helping them regain independence and confidence in their own homes.

Please note: This is a driving role. You must hold a full UK driving licence and have access to your own vehicle. You will be travelling between client homes throughout Swindon and the surrounding areas.

Key Responsibilities:

  • Provide high-quality care tailored to each individual's needs
  • Support with personal care, mobility, medication, and daily routines
  • Assist with light domestic tasks and meal preparation
  • Encourage independence during recovery at home
  • Mentor junior staff and support new starters
  • Work collaboratively with healthcare professionals

Requirements:

  • Minimum 1 years' experience in Health & Social Care (community experience preferred)
  • Full UK driving licence and access to your own vehicle
  • NVQ Level 2 (or equivalent) in Health & Social Care
  • Willingness to travel between client homes across the community
