We're currently looking for Support Workers to join our warm and welcoming Supported Living team. We offer both permanent contracts and flexible bank shifts to fit your lifestyle and availability.

Location: Swindon & surrounding areas

Pay Rate from: £12.21 per hour

Hours: Full-time, Part Time, Bank

Shifts: 12 hour days and/or Nights, 4-6 hour shifts (we operate a 24/7 service so timings will vary

About the role:
As a Support Worker, you'll be working with adults with a range of needs-this may include learning disabilities, autism, mental health challenges, or physical disabilities. Your role will be to support individuals to live full and meaningful lives in their own homes and local communities. You'll be delivering person-centred support, helping each person to achieve their own outcomes and live life on their terms.

This could include:
* Helping with day-to-day tasks like cooking, cleaning, budgeting, and personal care
* Supporting people to take part in activities they enjoy whether that's swimming, shopping, arts & crafts, music, attending groups, or going out for a coffee
* Encouraging independence and building confidence
* Supporting people to maintain relationships with family and friends
* Helping with appointments, medication.
* Promoting wellbeing and safety at all times


For some of our roles, we require at least 6 months' experience in a similar setting, though not all positions have this requirement. What's most important to us is that you have the right values, a caring attitude, and a willingness to learn. For those with less experience, we offer full training and ongoing support to help you succeed in the role.

About the Role:

This is a community-based role supporting individuals recently discharged from hospital, helping them safely transition back to independent living. You'll need a full UK driving licence and access to your own vehicle, as you'll travel between clients across Swindon.

As a trusted member of our care team, you will:

  • Deliver high-quality, person-centred care tailored to each individual
  • Assist with personal care, medication, and mobility
  • Support daily routines including meal prep and light domestic tasks
  • Promote independence and confidence after hospital discharge
  • Mentor and support junior/new staff
  • Liaise with healthcare professionals for the best outcomes

What You'll Need:

  • Minimum 1 years' experience in Health & Social Care (in a community based role highly desirable)
  • Full UK driving licence and access to your own car
  • Willingness to travel within the community
  • A minimum of a NVQ Level 2 or equivalent in Health & Social Care

The Role

In this role you will work in collaboration with the Quality Insite & Development Lead to support the continuous improvement of processes, and services by gathering insights, analysing data, carrying out colleague observations to ensure compliance, competency and strategies that enhance quality and efficiency. You will work within a multidisciplinary team to identify trends, asses performance and drive innovation.

This position is ideal for someone who has experience working within a Senior or Team lead role who is ready to take on a role focused on compliance and care quality standards.

Key responsibilities:

  • Collect, analyse, and interpret data to identify trends, performance gaps, and areas for improvement, supporting the development and implementation of quality initiatives.
  • Monitor system alerts, review care records, and ensure documentation aligns with regulatory and quality standards, including CQC compliance.
  • Collaborate with care teams to address care delivery gaps and escalate concerns appropriately to ensure timely resolution.
  • Maintain accurate documentation, contribute to audit reviews, and support the development of action plans to drive continuous improvement across services.
  • Assist in delivering training and development programmes aimed at enhancing competency and improving care quality standards.

What We're Looking For:

The ideal candidate will have a minimum of two years of hands-on experience in a care setting, demonstrating a strong foundation in providing quality support. They should hold a Level 3 qualification in Health and Social Care or be willing to work towards achieving it. Confidence and assertiveness are essential, along with the ability to challenge others constructively and professionally when needed. Excellent written and verbal communication skills are required, as well as proficiency in using a variety of computer systems and digital tools. The candidate should also be friendly, enthusiastic, and trustworthy, with a proactive, can-do attitude that contributes positively to the team and the people they support.
